FINALFORMS

We are very excited to announce that the Athletic Departments of Woodside and Summit Middle Schools have partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

With your own individual access, you may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking that ALL parents of athletes use FinalForms.  Homestead High School uses the same management system and the information will automatically be transferred after 8th grade.

 -------------------------------------- REMEMBER --------------------------------------

Physicals must be performed after 4/1/23.  Make sure to complete all forms on Final Forms - including parent/student signatures.  The medical professional who performs the physical must also sign the form (stamps are not allowed) and include their address, phone, license # AND date of exam. 

IHSAA Rule 3-10 requires that "Between April 1 and student’s first Practice in preparation for interschool athletic participation, the student shall have a physical examination by, or shall provide certification from, a physician holding an unlimited license to practice medicine, a nurse practitioner or a physician assistant, who shall clear the student for athletic participation using the current IHSAA Preā€Participation Evaluation form."

  • The physician's (PA or LPN) signature must be hand-written. NO signature stamps will be accepted!
  • The physician's (PA or LPN) signature AND license number must be on page 2.
  • If we receive an IHSAA physical with a stamped doctor's signature, it will not be accepted and considered invalid.

Practice Schedules

Athletes are expected to attend all practices. Individual problems/concerns should be brought to the attention of the coach prior to practice. Practice length should not exceed two hours (90 minutes is recommended). Practices on weekends and school breaks are not recommended. However, coaches may hold OPTIONAL practices over breaks and scheduled days with no school, such as President's Day, Teacher In-Service, etc.
 

Transportation
 

  •  All students will pay a $10 transportation fee for each sport.
  • School transportation will be provided to away events.
  • All students are expected to ride home after away events on the school bus, unless there is written documentation that the student will be riding home with their parent/guardian. 
  • Student athletes are expected to be picked up after all practices, games and meets promptly. If there is a pattern of late pick-ups from practice and/or games, alternate arrangements need to be made or it could result in dismissal from the team.
     

When will my child be finished and/or return home from an event?

 

  • To approximate a pick up time follow this formula:  Time event begins + length event (see below) + travel time from the competing location = Approximate pick up time.
  • Football: Approximately 1 hour and 15 minutes for each game. 7 th grade usually plays before the 8 th grade. (Total event time approximately 3 hours)
  • Cross Country: Approximately 1 hour and 30 minutes
  • Volleyball: Approximately 1 hour per game with 15 minutes between games. 7 th grade usually plays before the 8 th grade. (Total event time approximately 2 hours and 15 minutes)
  • Basketball: Approximately 1 hour per game with 15 minutes between games. 7 th grade usually plays before the 8 th grade. (Total event time approximately 2 hours and 15 minutes)
  • Wrestling: Approximately 1 hour and 30 minutes
  • Swimming: Approximately 2 hours
  • Track: Approximately 2 hours and 30 minutes
  • Soccer: Approximately 1 hour and 20 minutes per game

 

Student/Athlete Uniform Care

Athletes may be asked to provide or purchase a portion of the team uniform which they will keep at the conclusion of the season. Individual athletes will be responsible for the care and return of school equipment issued to them. Washing/care instructions should be followed. If none are given on or with the uniform; hand wash in cold water and air dry. Athletes will be expected to pay for lost or damaged equipment or uniforms. Please see the Athletic Director or School Treasurer for replacement costs.

 

Multiple Activity Participation
 

At the middle school level, participation in many activities is encouraged. However, if a student has extracurricular activities that overlap with school athletic schedules, that student should select one activity to avoid future conflicts. If a conflict arises between school athletics and a club or community team the athlete and parent must understand that the school team responsibilities take priority. A student may participate in more than one activity as long as it does not interfere with practice or game times.
